Affordable Care Act Survey
We want to hear from you! Share your story so we can make sure your voice is heard!
Many individuals and families that have been affected by cancer have had trouble getting the health care that they need and can afford. Recent health reform efforts will save lives from cancer and will help more people get the care they need when they need it.
If you or someone you know has:
- had trouble paying medical bills
- been denied insurance coverage for a health condition that you had before your coverage started
- skipped, canceled, or delayed an appointment, screening, or treatment
- had another problem with your health insurance coverage
Then please answer our brief survey by going to the following website:

Since the Affordable Care Act was signed into law on March 23, key provisions have already gone into effect that are meaningfully improving the health care system for cancer patients, survivors, and their families. By sharing your story, you are helping us to better understand how this law is already helping people.
All information will be kept confidential unless we have written approval to share it.
Improving access to quality, affordable health care is critical to winning the fight against cancer!
